Le occhiaie sono un’ombra scura, bluastra, sotto gli occhi. Dovute a fattori ereditari ed all’invecchiamento sono presenti a qualsiasi età. Non vanno confuse con le “borse sotto gli occhi” (grasso prolassato), correggibili solo chirurgicamente.

Cause più frequenti e trattamenti medici:

  • Pelle della palpebra inferiore molto sottile che lascia trasparire i vasi.
  • Vascolarizzazione eccessiva con importante fragilità capillare.

Occhiaia di aspetto bluastro conseguente a deposito dei pigmenti del sangue per rottura dei capillari (sfregamento degli occhi, stress termici), oppure a vasodilatazione con ristagno del sangue (fumo, alcool, ritenzione idrica da eccesso di sale, mancanza di sonno, ecc..).
Non ci sono trattamenti efficaci se la componente genetica è predominante. Il filler ci aiuta se presente anche una perdita di volume di grasso (“valle delle lacrime”).

  • Iperpigmentazione da accumulo di melanina, pigmento bruno prodotto per difendersi dagli insulti esterni. Occhiaia tendente al marrone. Costituzionale o secondaria ad insulti infiammatori (eccessiva esposizione solare, dermatiti, riniti allergiche, orzaioli, sfregamento degli occhi).

Difficile da trattare poiché le procedure possono risultare aggressive. L’ideale è un trattamento combinato:

– Applicazione domiciliare di prodotti depigmentanti realizzati dal farmacista solo dietro prescrizione medica poichè alcune sostanze, esempio l’idrochinone, sono vietate in cosmesi. Il trattamento è prolungato nel tempo, e può essere irritante.

– Peeling superficiale schiarente

Specifico per il contorno occhi e labbra è, ad esempio Enerpeel® El.Indicato anche per i giovani (dai 25 anni), come prevenzione dell’invecchiamento. Viene applicato con una particolare  penna.

– Laser e Luce pulsata.

– Biorivitalizzazione con iniettivi a base di antiossidanti.

Imperativa è la protezione solare totale dopo il trattamento e a lungo termine per evitare recidive.

  • Valle delle lacrime” o “tear through”

Accentuazione del solco lacrimale per perdita di volume del grasso sottocutaneo con suo scivolamento in basso. Legata all’invecchiamento ed al dimagramento eccessivo, o a fattori genetici.

Specifico per il contorno occhi è Redensity II di Teosyal. Mix di acido ialuronico, aminoacidi, antiossidanti, vitamine, minerali, che richiama poca acqua. Non consigliato, comunque, per chi soffre di gonfiori palpebrali.

Trattamenti generali preventivi

  • Protezione solare con creme 50+ ed occhiali da sole.

GAMBE PESANTI E GONFIE: RIMEDI

Alla sera dopo molte ore di lavoro in piedi o seduti, soprattutto durante la stagione calda, possono comparire disturbi alle gambe quali pesantezza, gonfiore delle caviglie. Il gonfiore è dovuto all’eccessivo accumulo di fluidi negli interstizi (edema). Raramente è sintomo di malattie sistemiche (insufficienza cardiaca, renale, epatica, ipotiroidismo, ecc.) soprattutto quando compare occasionalmente alla sera e si riduce alla fine della stagione estiva.

La più comune causa di gonfiore bilaterale alle gambe è l’insufficienza venosa degli arti inferiori, cioè un difficoltoso ritorno del sangue venoso verso il cuore. Sovrappeso, obesità e cellulite sono spesso presenti.
E’ proprio la stagione autunno-invernale quella adatta per intervenire in modo da contenere ed evitare l’aggravarsi della sintomatologia con l’arrivo del caldo.

Cos’è l’insufficienza venosa agli arti inferiori?

In stazione eretta o seduti il sangue venoso va dal basso verso l’alto, contro la forza di gravità; camminando le pompe muscolari del piede e del polpaccio spremono le vene ed il sangue procede verso l’alto. Le valvole delle vene impediscono al sangue di refluire in basso.

In caso di insufficienza venosa, il sangue rallenta nel ritorno verso il cuore (es. valvole difettose , ostruzione per trombosi, obesità e sedentarietà) , aumenta la pressione nelle vene (con dilatazione delle stesse = vene varicose), e nei capillari: da qui, i liquidi fuoriescono accumulandosi nei tessuti interstiziali (edema) dove non vengono sufficientemente drenati dal sistema veno-linfatico. Il caldo, determinando vasodilatazione, aggrava il ristagno ed accentua i disturbi.

Una visita angiologica, accompagnata da ecocolordoppler, può evidenziare ostruzioni venose, incontinenza valvolare e pianificare il trattamento delle vene varicose e dei capillari dilatati per ridurre il letto vascolare, aumentare la velocità del sangue e contrastare la stasi venosa.

RIMEDI PRATICI

La strategia consiste nell’ aumentare la velocità del ritorno venoso dagli arti inferiori al cuore, muovendo di più le gambe e controllando il sovrappeso.

  • Indossare calze elastiche a compressione graduale: elevata alla caviglia, decrescente salendo verso la coscia.
  • Camminare almeno 45 minuti al giorno.
  • Correggere le alterazioni della postura del piede (es piede piatto) con adeguati plantari, indossare scarpe comode con tacco 3-5 cm;
  • Evitare il sovrappeso e l’obesità che favoriscono il piede piatto, aumentano il carico delle vene.
  • Utile una dieta ipocalorica drenante.
  • Controllare la stitichezza, evitare indumenti stretti in vita che ostacolano il deflusso venoso;
  • Non stare fermi in piedi a lungo.
  • Da seduti, sollevare alternativamente il tallone e la punta del piede, ed alzarsi spesso.
  • Dormire con le gambe più in alto rispetto al cuore (rialzo sotto il materasso o meglio alzare i piedi del letto).
  • Non esporre le gambe al caldo, al sole nelle ore più calde: sconsigliati i bagni caldi, i fanghi, ecc.. Utili docciature di acqua fredda.
  • Non fumare. Evitare gli alcolici (vasodilatazione periferica);
  • Evitare, se possibile, farmaci che determinano vasodilatazione (es. antipertensivi, estroprogestinici, corticosteroidi, antinfiammatori non steroidei, alcuni antidepressivi)
